Does Insurance Cover HRT? Visits, Prescriptions, Memberships

Does insurance cover HRT for menopause? How plans treat the visit, the estradiol prescription and online program fees, plus the cash price when they do not.

By Julian Caraulani · Updated November 1, 2026

Often yes for part of it, rarely for all of it. Insurance can bill the clinician visit and, for FDA-approved estradiol filled at a pharmacy, the prescription, but program memberships and compounded products are usually cash. Estrogen at the online programs we track starts at $54 a month without insurance, and our HRT cost guide shows the full picture.

Coverage depends on your plan's drug list, deductible and network, so no guide can promise your copay. This one shows which bills insurance can touch and what the programs themselves say.

The three bills insurance can touch

Online HRT is not one purchase. It is three, and each is billed differently.

Bill Can insurance pay? What the programs say
The clinician visit Sometimes, if the program is in your network Found, Midi, Evernow and PlushCare bill visits to insurance
The medication Often for generic FDA-approved estradiol or progesterone filled at a pharmacy Evernow: insurance can cover most medications, but some ship only through its own pharmacy
The membership or subscription Rarely Evernow: membership fees are not insurance-covered

Programs that only sell cash-pay plans, such as Gala Health, do not bill insurance at all. Gala lists itself as cash-pay in our provider records, with the medication and provider review in one monthly price (from $69).

What each insurance-friendly program says

Quotes below were read on 1 November 2026.

Found. Found bills visits to your plan if it is in network: "If your plan is in-network we bill your visits to it and you pay the medication price." Without insurance "the visit is included" (Found). Its lowest all-in plan is from $40 a month without insurance.

Midi Health. "Midi is in-network with most PPO plans," and "Coverage, deductibles, coinsurance, and copays vary by plan" (Midi Health).

Evernow. "Evernow offers insurance coverage on all video visits, regardless of plan type." On drugs: "You can use insurance to cover the cost of most medications prescribed on Evernow," but "Some medications are only available for cash pay and must be fulfilled through Evernow partner pharmacies" (Evernow).

PlushCare. The page says "We can send prescriptions to any local pharmacy" and shows Humana, Aetna and Cigna among accepted plans (PlushCare). It charges a separate monthly membership on top of the visit.

Does Medicare cover HRT?

Medicare drug coverage (Part D) "helps pay for the brand-name and generic drugs you need" and is offered through private insurers (Medicare.gov). Each plan decides which drugs and pharmacies it covers, so check estradiol, patches and progesterone against your plan's drug list before you fill anything. Our Medicare HRT coverage guide goes deeper.

The harder gap is the visit. Midi says "Medicare is out of network with Midi" and "We're not able to accept Medicaid or Medi-Cal patients at this time" (Midi Health). Evernow says "We do not currently support coverage for Medicare and Medicaid plans" (Evernow). If you have Medicare, a cash-pay program that prescribes to your own pharmacy can still let you use your Part D plan for the drug.

When insurance does not help

  • Compounded hormones. These are usually cash. The FDA states "Compounded drugs are not FDA-approved" (FDA). See compounded vs FDA-approved HRT.
  • Cash-only programs. Program-dispensed medication is typically cash, though a prescription sent to your pharmacy may run through your plan.
  • Memberships. Evernow states "Any membership fees are not insurance-covered but can be HSA/FSA eligible."

How to check your own coverage

  1. Look up the drug (estradiol tablets, patch or vaginal cream) on your plan's formulary and note the tier.
  2. Ask whether the plan needs prior authorization or step therapy for it.
  3. Ask whether the program's clinicians are in your network, not just "accepted."
  4. Compare the insured total against the cash prices on our best programs page. Sometimes cash with a generic is cheaper than a copay plus a membership.

FAQ

Does insurance cover HRT?

It can cover parts of it. Visits at in-network programs can be billed to insurance, and generic FDA-approved estradiol or progesterone filled at a pharmacy may be covered by your drug plan. Memberships and compounded products usually are not. Your formulary, deductible and network decide the real cost, so check them first.

Does insurance cover online menopause programs?

Some programs bill insurance for visits, including Found, Midi, Evernow and PlushCare, while others are cash-only. Even at insurance-friendly programs, the membership or some medications may be cash. Confirm that your specific plan is in network before booking, since acceptance of "most PPO plans" does not mean yours is included.

Does Medicare cover hormone replacement therapy?

Part D plans can cover estradiol and other drugs, depending on each plan's drug list. Several online programs do not accept Medicare for visits, though. Check your plan's formulary for the exact product, and consider a cash-pay visit with a prescription sent to your own pharmacy.

Is compounded HRT covered by insurance?

Usually not. Compounded hormones are not FDA-approved and are most often sold cash-pay through the program's partner pharmacy. FDA-approved generics, such as estradiol tablets or patches, are the products most likely to be covered by a drug plan.

Can I use an HSA or FSA for HRT?

Evernow says membership fees "can be HSA/FSA eligible," and our records show Hone Health accepts HSA/FSA. Eligibility rules differ by plan administrator, so confirm with yours before paying.

Is cash cheaper than using insurance?

Sometimes. A generic estradiol prescription can cost little at a pharmacy, but an insured program may add a membership plus a visit copay. Compare the total on our price table against your plan's copays.
