Does Cigna Cover HRT for Menopause? Estradiol Policy Guide
Does Cigna cover hormone therapy for menopause? What Cigna's own step therapy and quantity policies say about patches, gel and pellets, plus the cash fallback.
By Julian Caraulani · Updated November 5, 2026
Often yes for generic estradiol, with rules. Cigna's National Formulary policy names generic estradiol patches as its "Step 1" product (Cigna), and compounded pellets are not covered. Your employer plan decides the final answer. Cash generic estradiol starts at $54 a month at the programs we track.

Cigna publishes its drug coverage policies as PDFs. This guide reports what three of them say, each read on 5 November 2026. They describe Cigna's standard approach; the Cigna document itself warns that a customer's plan document "ALWAYS SUPERSEDES" it, so check your own formulary.
What Cigna says about estradiol patches and gels
The Estrogen Transdermal Step Therapy Policy (CNF094, review date 07/05/2023) sets an order of products:
| Step | Products |
|---|---|
| Step 1 | Estradiol patches (generic) |
| Step 2 | Elestrin, EstroGel, Evamist, Divigel, estradiol 0.1% gel |
The policy states: "If the patient has tried one Step 1 Product, approve a Step 2 Product." It also approves a Step 2 product if the patient has tried brand Climara, Minivelle or Vivelle-Dot patches. Approvals "are provided for 1 year in duration."
In plain terms, if you want gel or spray on a Cigna plan that uses this policy, expect to try a generic patch first, or have a prescriber request an exception. Our estradiol patch guide covers the products, and the patch side effects guide covers skin reactions that sometimes lead people to switch.
Cigna's policy adds that transdermal estrogen "may be less likely to produce thrombotic risk" than oral estrogen (Cigna). Separately, the Menopause Society puts the evidence more carefully: "Observational studies have not demonstrated an increased risk of VTE with transdermal ET, and limited observational data suggest less risk with transdermal versus oral ET, but comparative RCT data again are lacking" (The Menopause Society, 2022).
Quantity limits on patches
Cigna's Estrogens (Topical) Patches Drug Quantity Management Policy (review date 01/05/2026) says it was developed "to prevent the stockpiling, misuse, and/or overuse of the estrogen patch products." Limits apply per 28 days and per 84 days, and the policy lists override criteria, with approvals "for 1 year in duration, unless otherwise noted." If a pharmacy rejects a refill for quantity, your prescriber can ask for an override. With patch supply uneven in 2026, see the estrogen patch shortage guide.

Compounded hormones and pellets
Cigna's Compounded Medications policy (IP0251, effective 12/15/2025) states: "Compounded Hormone pellets are considered to be experimental, investigational or unproven" (Cigna). Estradiol, estriol and estrone pellets are named. Separately, the FDA states "Compounded drugs are not FDA-approved" (FDA). The policy also quotes ACOG: "Conventional hormone therapy is preferred over compounded hormone therapy given the available data." If a program sells compounded products, expect to pay cash. See compounded vs FDA-approved HRT.
What we could not confirm
- Tier and copay for any specific plan. Those sit in your formulary, not in these policies.
- Vaginal estradiol cream, tablets and rings. We did not find a Cigna policy page for them that we could quote.
- Whether a given telehealth program bills Cigna. Check the program page or our reviews.
If your plan does not cover it
Generic estradiol is often cheaper in cash than a deductible-phase copay, so compare both. Our top-scored program, Winona, lists $39 a month. See does insurance cover HRT for the wider picture and how much HRT costs online.
FAQ
Does Cigna cover hormone therapy for menopause?
Cigna's National Formulary policies set rules for estradiol patches and gels, which indicates they are covered products under standard plans, subject to step therapy and quantity limits. Your employer's plan document controls the final answer, so check your formulary or call the number on your card.
Does Cigna cover HRT?
For FDA-approved estradiol patches and gels, Cigna's policies describe coverage with conditions. Compounded hormone pellets are classed as experimental, investigational or unproven. Tier and copay depend on the plan, so look up each product by name in your formulary.
Does Cigna cover the estradiol patch?
Generic estradiol patches are the Step 1 product in Cigna's transdermal step therapy policy, and a separate policy sets quantity limits per 28 and 84 days. Override criteria exist, so ask your prescriber to request one if a refill is rejected.
Does Cigna cover estradiol gel?
Gel is a Step 2 product. The policy approves it if you have tried one generic Step 1 patch or a brand patch. Without that history, the pharmacy may reject the claim and your prescriber can submit an exception request.
Does Cigna cover hormone pellets?
No under the policy we read. Cigna's IP0251 policy lists compounded testosterone and estrogen pellets as experimental, investigational or unproven, so plan on cash payment. Our HRT cost guide shows what FDA-approved options cost instead.
Does Cigna cover online menopause programs?
It depends on the program. Some bill insurance for visits and some are cash only. Cigna's drug policies do not address telehealth billing, so confirm with the program before booking.
