Online hormone replacement therapy means a licensed clinician reviews your history by form or video and, if you are eligible, prescribes hormones that ship to you or go to your pharmacy. How hormone replacement therapy online works, step by step
Most programs follow the same path, though the details differ.
- Intake. You answer questions about symptoms, periods, medical history, medicines and family history. Some programs ask for recent blood pressure or lab results.
- Clinician review. A licensed clinician reads your answers, and sometimes talks to you by video or message. Found says its clinicians are "Licensed clinicians, prescribed online" (Found). PlushCare says you can "consult with one of our board-certified doctors online today" (PlushCare).
- A decision, which can be no. Found states that "Prescriptions are up to a medical provider's discretion" and offers a "Full refund if you're not clinically eligible" (Found). Being turned down is a normal outcome, not a failure of the service.
- Fulfilment. Medication ships from a partner pharmacy, or a prescription goes to the pharmacy you choose. Our state guide shows where each program says it operates.
- Follow-up. Good programs check in on symptoms and side effects and adjust the plan. Ask how often, and whether messaging is included in the price.
Dosing is a clinician decision, so this page gives none.
What the evidence and the FDA say
The Menopause Society's 2022 position statement says systemic estrogen therapy in a woman "with an intact uterus using systemic ET should receive adequate progestogen" (The Menopause Society). It also notes that "Transdermal estradiol and oral CEE are similarly effective in alleviating VMS" (The Menopause Society). VMS means hot flashes and night sweats.
On labeling, the FDA announced it is working to remove several boxed warnings from menopausal hormone products, but it is "not seeking to remove the boxed warning for endometrial cancer for systemic estrogen-alone products" (FDA). The same release says the labeled recommendation will be to "start HRT within 10 years of menopause onset or before 60 years of age for systemic HRT" (FDA). Whether any of this applies to you is for your clinician to judge.

FDA-approved or compounded: ask before you pay
Some online programs sell FDA-approved estradiol and some sell compounded products. On compounding, the FDA says "Consumers who purchase compounded drugs online, such as through online pharmacies and telehealth platforms, may not know the identity of the compounder that produced the drug" (FDA). Our guide to compounded versus FDA-approved HRT explains the difference. Always ask which type you are buying.

What online HRT costs and what insurance does
Cash prices vary by product and by what is bundled. Some programs bill visits to insurance; others charge for the visit and leave the medication to your pharmacy. Our HRT cost guide breaks down the parts, does insurance cover HRT covers plans, and the live price table shows current numbers.
When online HRT is not the right route
Online care suits straightforward cases. Some situations call for an in-person visit: unexplained vaginal bleeding, a personal history of breast cancer or blood clots, or symptoms that do not match menopause. A good program will say so and send you to in-person care. If a site promises a prescription before any clinician has looked at your history, treat that as a warning sign and use our scam checker.
FAQ
Hormone replacement therapy online: is it legitimate?
Yes, when a licensed clinician reviews your history before prescribing and the medication comes from a licensed pharmacy. Check who prescribes, where the drug comes from and whether the product is FDA-approved or compounded. Our reviews and scam checker cover individual programs.
How does HRT online work?
You complete an intake, a clinician reviews it and may talk with you, and if you are eligible a prescription is sent to a partner or local pharmacy. Follow-up messaging adjusts the plan over time. The steps above show where programs differ.
Do I need a video visit to get HRT online?
It depends on the program. Some use a written intake reviewed by a clinician, and others schedule a video or phone visit. Rules also vary by state. Ask before you pay, and check the program's own page for how its first review works.
Can online HRT be cheaper than seeing my own doctor?
Sometimes. A program with one all-in monthly charge can cost less than a visit plus a separate pharmacy fill, but with good insurance your own doctor and generic estradiol may cost less. Compare using the live cost page and your plan's formulary.
What will the clinician ask me?
Expect questions on symptoms, periods, and past blood clots, cancers and liver disease. They will also ask about family history, current medicines and whether you have a uterus. Answer fully, because that review decides what is safe to prescribe.
Is a high-ranked program a medical recommendation?
No. Rank reflects price clarity, care model, access and reputation as published by each program. Whether a hormone suits you is a decision with a clinician, and a revised medical history can change the answer.
